Ceftobiprole is a broad-spectrum cephalosporin with potent in vitro and in vivo activity against a range of Gram-positive and Gram-negative pathogens. In vitro, it demonstrates strong activity against methicillin-resistant Staphylococcus aureus (MRSA), methicillin-susceptible S. aureus (MSSA), Streptococcus pneumoniae, and Enterococcus faecalis, with MIC50/90 values of 0.25/2 mg/L against E. faecalis. However, its efficacy against Enterococcus faecium is limited, with MIC90 values exceeding 32 mg/L. Against Gram-negative pathogens, ceftobiprole exhibits activity comparable to cefepime, with MIC90 values of 0.12 mg/L against ceftazidime-susceptible Enterobacteriaceae and 8 mg/L against Pseudomonas aeruginosa. In vivo, ceftobiprole has shown efficacy in various animal models. In murine skin infection models, it significantly reduced bacterial load in both MSSA and MRSA infections when administered as the prodrug ceftobiprole medocaril.
