Vadadustat is a small-molecule inhibitor of hypoxia-inducible factor (HIF) prolyl hydroxylase (PHD) enzymes, which regulate HIF stability. In vitro, vadadustat stabilizes HIF-α subunits, promoting transcription of HIF-responsive genes, including erythropoietin, at nanomolar concentrations. In vivo, oral administration in animal models of anemia and chronic kidney disease increases circulating erythropoietin levels, stimulates red blood cell production, and improves hemoglobin levels without significant toxicity, demonstrating robust erythropoietic activity.
