Vericiguat is a soluble guanylate cyclase (sGC) stimulator that enhances cyclic GMP production in cardiovascular tissues. In vitro, it directly stimulates sGC in the presence or absence of nitric oxide, increasing cGMP levels in vascular smooth muscle cells and cardiomyocytes at nanomolar concentrations. In vivo, oral administration in rodent and canine models of heart failure improves cardiac output, reduces left ventricular end-diastolic pressure, and lowers vascular resistance at doses of 0.3–10 mg/kg, demonstrating improved hemodynamics and protection against cardiac remodeling.
