Ibrexafungerp (SCY-078) is a first-in-class orally bioavailable triterpenoid antifungal that inhibits β-(1,3)-D-glucan synthase, disrupting fungal cell wall synthesis. In preclinical studies, it showed potent fungicidal activity against Candida spp. (MIC90 values 0.12–0.5 µg/mL) and fungistatic activity against Aspergillus spp. (MIC90 0.5–1 µg/mL), including strains resistant to azoles and echinocandins. In murine models of disseminated candidiasis, oral ibrexafungerp significantly reduced fungal burden in kidneys and prolonged survival. It also demonstrated activity in models of invasive pulmonary aspergillosis. Pharmacokinetic studies revealed good oral bioavailability and tissue distribution, supporting its development as a broad-spectrum antifungal for invasive and mucocutaneous mycoses.
