Fostemsavir is an oral prodrug of temsavir, a first-in-class HIV-1 attachment inhibitor that selectively binds to the viral envelope glycoprotein gp120. In vitro, it prevents conformational changes required for CD4 receptor binding, thereby blocking viral entry into host cells across diverse HIV-1 subtypes. In vivo, fostemsavir demonstrates potent, durable suppression of plasma HIV-1 RNA in treatment-experienced patients with multidrug-resistant infection, with an acceptable safety and tolerability profile.
