Iomeprol is a nonionic, low-osmolar iodinated contrast agent primarily used for X-ray and CT imaging, and its bioactivity is characterized mainly through its physicochemical, pharmacokinetic, and tolerability profiles rather than receptor-mediated pharmacology. In vitro studies show that Iomeprol exhibits low cytotoxicity toward human endothelial and renal tubular cells, with cell viability remaining above 80% at concentrations up to 100 mg I/mL, indicating minimal cellular toxicity compared to older ionic agents like diatrizoate. It also shows low protein binding (< 5%) and minimal interaction with erythrocyte membranes. In vivo (animal) studies in rats, rabbits, and dogs demonstrate rapid renal excretion (> 90% within 24 h), low systemic toxicity (LD₅₀ > 20 g I/kg i.v. in rodents), and negligible cardiovascular effects at diagnostic doses (1–2 g I/kg). In renal safety models, Iomeprol induces only mild, transient tubular vacuolization at very high doses (≥ 5 g I/kg), far less severe than that caused by older agents, confirming its favorable safety and tolerability profile in preclinical settings.
