Finerenone (BAY 94-8862) is a novel, selective non-steroidal mineralocorticoid receptor (MR) antagonist. In preclinical studies, it bound the MR with high affinity (Ki ~18 nM) and demonstrated greater selectivity versus other steroid hormone receptors compared to spironolactone and eplerenone. Finerenone inhibited aldosterone-induced MR signaling, reducing pro-fibrotic and pro-inflammatory gene expression in cardiac and renal cells. In rodent models of hypertension and heart failure, it decreased cardiac hypertrophy, fibrosis, and proteinuria at doses that were well tolerated. Pharmacokinetic studies showed good oral bioavailability and balanced distribution between heart and kidney tissues, supporting its progression into clinical development for cardiorenal diseases.
